Dear Caregiver:
Cook Children’s needs your help with the following confidential survey about children’s health in our area. We regularly conduct a Community Health Needs Assessment (CHNA) to identify the children’s health needs in our community and develop solutions with community partners to address the most important issues. We need information from caregivers of children ages 0-17 in the following counties: Collin, Denton, Grayson, Johnson, Hood, Parker, Tarrant, and Wise counties.

We invite you to take a few minutes to complete this private survey in the next few days. It is a simple way that you can give back to your community and make our region an even better place to live. Your participation is needed even if you have never used Cook Children’s services.

We will add your responses to this private survey to responses received from other caregivers. With information from caregivers throughout the region, we can identify the strengths and challenges to make our community the healthiest place to raise children.
